I understand that my child must be fully potty trained by the first day of school. He/She must be able to recognize the need to use the bathroom and be able to dress and un-dress independently for bathroom use. Accidents do happen, and in such cases, the parent/guardian will be notified.*

Preferred class time not guaranteed. Time preference will be granted on a first come, first served basis and will be based on availability. Your spot and time are reserved once this document is submitted AND the registration fee is paid in full.

I agree to pay the monthly tuition of by the first of the month for each month of the school year (Sept-May). Tuition is paid in advance for the upcoming month and it is non-refundable. Tuition is payable by cash, check (made out to Katie Springer), Cash App ($katiespringer33), or Apply Cash (801-380-7516). If tuition is not received by the 5th of the month, a late fee of $5 will be added to that month’s tuition. If tuition is not received by the 15th of the month, an addition late fee of $15 will be added to that month’s tuition. If tuition is not paid in full by the last day of the month, Miss Katie reserves the right to suspend the child until all tuition and late fees have been paid, to ask for several months of advance payment, or to terminate the enrollment of the child in Katie’s Korner Preschool.*
